Leveraging RTLS for Life-Safety and Beyond: Why 433 MHz RF Is the Gold Standard

“Few things are more stressful than a 3AM call during a winter freeze that an Alzheimer’s patient has gone missing and nobody knows for how long or whether the patient is inside or outside the facility.”

If your organization is considering a campus-wide Real-Time Locating System (RTLS) to enhance patient safety, staff protection, or operational efficiency, understanding the technology behind indoor positioning is critical. Not all RTLS solutions are created equal, and the frequency your system operates on determines whether it will perform reliably when lives are at stake.

With over three decades of field-tested deployments across VA medical centers, hospitals, and correctional facilities, MGM Solutions has learned that 433 MHz RF should be your primary location engine for any life-safety RTLS application.

The Evolution of Indoor RTLS Technology

RTLS technology has evolved significantly since MGM first introduced it for tracking patients and staff in Emergency Departments in the early 1990s. Here’s how the technology progressed:

Why 433 MHz RF Is the Gold Standard for Life-Safety

After testing virtually every frequency and technology available, MGM’s patented triple-technology approach centers on 433 MHz RF as the primary location engine. Here’s why:

What About Higher Frequencies?

Higher frequencies like 2.4 GHz (Wi-Fi, BLE) and 800-900 MHz have proven unreliable for life-safety applications. VA Medical Center testing showed that these frequencies produced locations on incorrect floors or over 100 feet from actual position. When VA Police require 25-foot accuracy and a patient’s life may depend on rapid location, that level of error is unacceptable.

SecurTRAK’s Triple-Technology Architecture

SecurTRAK combines three complementary technologies into a single, unified platform:

1. 433 MHz RF Transmitters

The backbone of the system. RF signals pass through walls, doors, and equipment to provide continuous real-time location across entire campuses — indoors and outdoors.

2. 125 KHz Low Frequency (LF) Exciters

Mounted at portals and chokepoints, LF exciters create adjustable electromagnetic bubbles (2-10 feet diameter) that detect tags within milliseconds. Each exciter has a unique ID linked to a specific location, providing precise floor-level and directional data. They can penetrate up to 2 feet of concrete.

3. Infrared (IR) Readers

Where line-of-sight is available, IR provides room and bed-level granularity for applications like automated clinical rounding and infant identification.

Building System Integration

SecurTRAK’s LF exciters, RF, and IR readers feature managed inputs/outputs that interface directly with building systems:

Vector-Based Floor Plan Display

Early RTLS systems struggled with managing 250,000+ square foot facilities that required 1,000+ raster images. MGM solved this by integrating with clients’ AutoCAD floor and site plans using vector format, delivering:

Applications Beyond Patient Tracking

Once the SecurTRAK infrastructure is deployed, the same platform supports numerous additional use cases:

Design Considerations for Your RTLS Deployment

  1. Define all use cases upfront — collaborate with every department
  2. Design for the most demanding use case — this sets infrastructure requirements
  3. Conduct RF heat-mapping — ensure zero dead zones across the campus
  4. Plan IR coverage strategically — only where room/bed-level granularity is needed
  5. Build for scalability — the right infrastructure grows with your needs
